Our aftercare planning program

Recovery doesn’t end when treatment does.

Treatment is the core of recovery, but addiction is a lifelong disease. The full process moves through stages: accepting the need for help, detox, treatment, and aftercare. Without all four, the cycle of relapse keeps coming back.

Aftercare is the most often overlooked step — and the most crucial for keeping the work going once treatment ends. At Northridge Recovery, every client leaves with access to aftercare services. Our goal is not short-term recovery, but lifelong sobriety.

What is aftercare?

The plan that begins where treatment ends.

Aftercare is a collection of services offered to clients once they’ve completed treatment. It extends the benefits of the program well beyond its closing date — so clients can keep working on the underlying issues while they rebuild their lives.

What aftercare looks like varies by person and program. Done well, it can help with everything from regaining what addiction has taken to repairing relationships, and it can help families learn to live alongside a loved one in recovery.

At Northridge Recovery, we believe aftercare is the key to long-term success. As clients exit our program, we work with them to choose the services they need to continue down the road to sobriety. Not every option fits every person — we have a wide range available.

After rehab

What happens after I complete rehab?

Aftercare isn’t one program — it’s a set of options that fit different lives. Common forms include:

Sober living

A bridge between treatment and home.

Some clients move into a sober living facility after rehab. They can leave the facility with curfews in place, continue group therapy, and learn job-readiness skills for life after recovery.

Outpatient care

Therapy alongside daily life.

Three types — partial hospitalization (6–8 hours/day, family at night), intensive outpatient (a few visits weekly), and standard outpatient (one or two visits weekly).

Community & events

Group activities throughout the year.

Barbecues, get-togethers, group therapy meetings — weekly, monthly, or seasonal. Especially helpful for clients without strong family support.

Aftercare also includes education and job resources, housing, mentor and advisor relationships, and 12-step programs like AA and NA — designed to put people in recovery alongside others walking the same road.

There’s help and support for families too. In family groups, members can share experiences, learn what to expect, and figure out how to rebuild relationships and support a loved one’s recovery.
